Camp health forms
All campers must fill out and bring state-required health and immunization record forms.
These may be mailed, e-mailed or brought with you on the first day of camp. You may be able to get a copy of your child’s physical exam/health form and immunizations from their school nurse before the end of the school year. If you have questions don’t hesitate to contact us.
